This survey is for people who access services at Self Help, CMHA WWD. Self Help services are offered by people and for people who have experienced a mental health and / or addiction issue. We believe recovery is possible!

By participating in this survey you are agreeing to participate in this evaluation of Self Help Alliance services. Confidentiality will be maintained to the degree permitted by the technology used. No absolute guarantees can be made regarding the confidentiality of electronic data. This survey does not ask for any identifying personal information and the Self Help staff collecting survey data will not be aware of who completes this survey. Your responses will remain anonymous.
